All Division 4 competition travel teams must report all their league match scores.

• Home teams must report each league match score.
• Away teams must report each league match score.
• Winning teams must report each league match score.
• Losing teams must report each league match score.
• All age groups, including 9U, 10U, Boys and Girls must report each league match score.
• Both teams involved in a postponed game must report the postponed game.

The following do not report or are not reported to the district using this form:

• Recreation teams (division 5) do not report.
• Teams in state premier leagues do not report to the district.
• Teams only report league matches and not tournaments or friendlies.
